Skin Conditions - Form required

Identifying skin conditions such as ringworm, early is crucial to prevent their spread among team members and to ensure the health and safety of all athletes. Promptly notifying a coach or program director allows for immediate action to minimize exposure to others and helps maintain a safe environment. Seeking medical attention right away not only ensures proper diagnosis and treatment, but it also enables the athlete to begin the necessary steps to return to practice and competition with a doctor's clearance, reducing the risk of extended absences and further complications.

If your wrestler experiences a skin condition (including eczema) during the season, please ensure this form is completed by a physician.  This will need to be presented to coaches at practice, as well as the head official of any tournament.  Make additional copies since sometimes referees keep the form when you present it at skin check.  Finally, this form is only good for two weeks after the date you begin treatment.  If the condition is still visible, you will need to see the doctor again and have another form filled out.
